What To Bring
Here's what you'll need.
General income – Form W-2 for employment, Form 1099-MISC. for self-employment, Schedule K-1 for active or passive partners.
Investment Income – Form 1099-INT for bank interest, Form 1099-DIV for dividends and Form 1099-B for stock sales
Property deduction – Form 1098 mortgage interest, property taxes paid, HUD-1 or closing statement for new owners
Education credit – Form 1098-E student loan interest and Form 1098-T tuition paid
Dependents – We the need name, federal ID (social security number) and address for each child care provider or after school program (summer camp). Also please make sure you have with you the date of birth and social security number for any new dependent you want to add to your return.
Donations – All donations by cash/check and non-cash (i.e. Salvation Army receipts)
To speed up your appointment please send us your tax forms and other documents 10 days in advance. You could do that via email (preferred), fax or regular mail. Emails go to firstname.lastname@example.org If possible scan everything as one file and name it in the following format LastName,FirstName-Date-Of-Appointment (Doe,John-2-10-13). If the email is too big you can scan in a lower resolution, zip the file(s) or use www.hightail.com uploading service. Make sure to list the date and time of your appointment in the subject line. When you fax to 917-386-2568 make sure you put on the cover sheet the date and time of the appointment and the number of pages on the cover sheet. If you decide to send your forms with regular mail or a messenger service make sure they are only copies.
As always please bring all originals with you on the day of your appointment and if you haven’t filed 2012 with us bring a copy of that return. You don’t need to send us your business expenses in advance, just add them up with totals for the year by categories. If you need help ask us for a template spreadsheet. If you have any stock sales please make sure you the cost basis for all of them.
